Custom Forms

What it does

Lets you build your own forms — surveys, contact forms, applications, registration pages, feedback questionnaires — without touching code. Drag in fields, organize them into sections, design how it looks, publish it as a public page, and watch the responses roll in. You can share the form by URL, embed it on your site, or print a QR code.

Who can use it

  • Anyone in your organization with read permission for the Custom Forms module.
  • Creating, editing, and deleting forms needs the matching permission. Without it, Create Form is greyed out.
  • You must verify your email before you can create your first form.

From the sidebar, open Marketing and click Custom Forms.

Step by step

Create a new form

  1. On the Forms page, click Create Form in the top-right.
  2. Pick a starting point:
    • Use a template to start from a pre-built form (contact form, NPS survey, application, etc.). The template's sections and fields are created for you.
    • Blank form to start from scratch.
  3. In the form-details dialog, fill in:
    • Form Name — for example, "Customer Feedback Survey". Up to 100 characters.
    • Description (optional) — shown at the top of the form when people open it. Supports rich text.
    • Header Image — upload an image to display as a banner at the top of the public form.
  4. Click Create Form (or Create from Template if you picked one). Nautis opens the form builder.

Add sections and fields

The builder has two main areas: Builder (where you add fields) and Live Preview (which updates as you type).

  1. In the Builder tab, click to add a section. Give it a name (for example, "About You") and an optional description.

  2. Inside a section, add fields. Choose a field type from the picker:

    Basic

    • Text Field — single line, for names, titles.
    • Textarea — multi-line, for longer answers.
    • Email Address — with email validation.
    • Phone Number — with phone formatting.
    • Number Input — with min/max validation.

    Input

    • Date Picker — for birthdays, deadlines.
    • Date & Time — combined date and time.
    • URL Field — for website links.
    • Range Slider — for ratings, budgets.
    • Color Picker — hex input with presets.
    • Currency — currency-formatted number.

    Selection

    • Select Dropdown — single choice.
    • Multi-select Dropdown — multiple choices.
    • Radio Buttons — single choice, visible.
    • Checkboxes — multiple choice, visible.
    • Toggle Switch — on/off, yes/no.

    Upload

    • File Upload — documents, PDFs.
    • Image Upload — with crop.
    • Video Upload — for portfolios or demos.

    Advanced

    • Rich Text Editor — formatted text input.
    • Rating — star, NPS, or emoji rating.
    • Signature — digital signature pad.
    • Address — composite street/city/state/zip/country.

    Layout

    • Heading, Paragraph, Divider — display-only, for structure.
  3. For each field, fill in its Title, Placeholder, whether it's Required, and any options (for example, the choices on a dropdown).

  4. Drag fields up and down to reorder. Drag a field from one section to another to move it.

The right-hand Live Preview shows exactly what people will see — click View Form to open the full public version in a new tab.

Design how the form looks

  1. In the form builder, switch from the Builder sub-tab to the Design sub-tab.
  2. Pick fonts, colors, button styles, and spacing. You can also turn on Stepper Mode, which splits long forms into a step-by-step wizard.
  3. Save. The live preview updates immediately.

Publish your form

  1. In the form-builder header, flip the Publish toggle on. The button label changes to Published with a green check.
  2. Once published, Nautis exposes the public form URL. The header now shows a green Live indicator.

You can't publish an empty form — at least one active field is required.

Share or embed the form

In the form builder, click Share in the header. The popover gives you four options:

  • Form URL — the full public link. Click the copy icon to grab it.
  • Embed Code — paste this HTML into your website to embed the form inline.
  • Short URL — click Generate Short URL to create a clean, branded short link.
  • QR Code — preview and click Download QR Code to save a PNG you can use on flyers, slides, or print materials.

Anyone with the form URL can fill it in — no Nautis account needed.

View and manage responses

  1. From the Forms list, click the entries icon on a form (or open the form and click the entries link in the header).
  2. The Entries page shows:
    • A Statistics panel with totals and trends.
    • The Entries Table with one row per submission.
  3. Use the search bar to find a specific entry, or click Advanced Filter to filter by field value, date range, or submitter.
  4. Click any row to see the full response in a side panel.

Export entries

  1. On the Entries page, open the export menu in the statistics panel.
  2. Pick CSV or JSON.
  3. To export a subset, tick the rows you want first, then use the bulk-export menu in the table.

The download starts straight away.

Edit a published form

You can keep editing a form after it's published — your changes go live the moment you save. People who've already submitted won't be re-prompted; their old responses stay as they were.

To unpublish, click Published in the header to toggle it back to Publish.

Tips & limits

  • Your plan caps how many forms you can create each month. When you hit the limit, Create Form opens a "Custom Form Limit Reached" modal showing your current count and limit. Upgrade your plan to add more.
  • Form names must be under 100 characters; descriptions under 2000.
  • File uploads are stored against your organization's storage quota.
  • Use Sections to group related fields and reduce visual overwhelm on long forms.
  • Stepper Mode is great for applications and onboarding — it shows one section at a time with a progress bar.
  • The Header Image sets the tone for the form before anyone reads a question.
  • For surveys, the Rating field supports stars, NPS (0–10), and emoji scales — pick whichever fits.

Frequently asked questions

Do people need a Nautis account to fill in my form?

No. The form is public. Anyone with the link can submit, and you'll see every entry on the Entries page.

Can I make a field required?

Yes. Open the field's settings and toggle Required on. The public form will block submission until that field has a value.

Can I reorder fields after I've added them?

Yes — drag and drop in the builder. You can also drag a field from one section to another.

What happens to responses if I delete a field?

Existing responses keep the old data, but the field no longer appears on the form or in the export. To be safe, export your entries before deleting fields.

Can I have more than one form?

Yes — up to your plan's monthly cap. Each form has its own public URL, theme, and entries.

My form won't publish — why?

Forms need at least one active field to publish. Add a field, then try again.

How do I let people upload files in their answer?

Add a File Upload, Image Upload, or Video Upload field. Uploads count toward your organization's storage quota.

Can I generate a QR code?

Yes. Click Share in the builder, scroll to QR Code, and click Download QR Code to get a PNG.
